Adding text to request templates

IT would be nice to have the ability to add some text to a service request/ incident template to give some help to users who may not understand what the field is for. For ex. on many forms you have a little icon beside the field that when click, a little box pops up giving some context/help on the field.

It would also be nice to be able to add some text in the template that is not associated with a field label. This  would help to organize more complex forms.


For example we have a paper form that users have to fill out to get VPN access. I want to create an electronic form and can use tools like Adobe Acrobat or create a sharepoint form. Since users are contacting the help desk find out how to get VPN access, instead of sending them elsewhere I wanted to create a request template that is essentially the form. For that I need to be able to add some text to explain the different sections. This would help in many ways. I can confirm who the person is who filled out the form as they have to log in to SDP to fill out the request. I can add this to the service catalogue and everything is done in one step.
