We have two approved requests in the servicedesk. I need a sql report where we can track the approval status of two-step requests.

The report can list the last month's data

Request ID, Requester, Service Category, Site, Group, Reqesut Statu, Approved Statu, Technicians, The first stage approval, First Stage Approval Status,  Second Stage Approval, Second Stage Approval Status,
