Create Notes from a Worklog

I'd like to be able to automatically create a note from a worklog entry.
 
It's important to track both time and the actions taken by a technician as a distinct entry with a timestamp.  The worklog presently tracks the time, and the note displays a note about the actions to other techs working on the issue.
 
How about adding a checkbox that would add a note based on the information typed into the worklog?
