Incident Dropdowns Not Showing Up

We currently have ServiceDesk 9.0 Build 8991 installed and working on a Windows 2008 R2 virtualized server. We had the system running and started to put subcategories into the New Incident Default Request Template. When we create a New Incident and select Category, the subcategory shows no additional added items other than Select Subcategory as shown in the attachment. We have verified that there are three subcategories under the AX Dynamics category, yet they still do not show up. Anyone familiar with this or know a solution to fix it?
