New user request form and workflow

I'm just wondering if there is a standard/supported/best practice way of creating a New User Request electronic form and accompanying workflow, to automate AD user account creation entirely within SD+.

In essence, I'd like for certain Requesters (Managers, Department Heads etc) to have access to a New User Request form within SD+ that contain the standard fields necessary to create a new user. Ideally, this would then be submitted as a ticket and assigned to a technician who would skim over it quickly for any obvious errors, before actioning it. It would then trigger a workflow that would create the AD account using the details provided.

I'm currently using a cobbled together form to complete this, which outputs to its own database that needs to get checked regularly by technicians, who then manually create user accounts. Not good. I would LOVE for this to be thrown out, and instead tightly integrated into SD+.
