Powershell result :The operation couldn't be performed because object ' Users LDAP Path' couldn't be found on ' My Domain Controller'.

I reviewed a few other new user mailbox creation requests and did not have this issue. I was wondering if there was a way to diagnose if this was a one off error. I cannot go live with this tool in production until I can assure my team that this will not be an issue going forward.
