We have following in REPLY template (Re: [ID $RequestId] : $Title)
I think it's standard.

From second reply we have adding new RE:[ID ***] Every reply it's added
What could be the problem?

As example:
RE: [ID 54432] : RE: [ID 54432] : RE: [ID 54432] : RE: [ID 54432] : Graph reques
